Call 225-342-8536 to obtain test dates and times.<\/p>\n<p>This position is assigned to the Center for Preventive Health Services in the Office of Public Health (OPH) and serves as the Program Monitor for communities and\/or sponsoring agencies that operate School-Based Health Centers (SBHCs).\u00a0 The Program Monitor is responsible for training school health center sponsors on OPH policies, procedures, planning and implementing SBHCs, as well as being an integral participating member of the Adolescent School Health Center Network.\u00a0 The Network is a group composed of the sponsors and staffs of operating school health centers funded by the Maternal and Child Health Block Grant and other federal, state, and local funding sources.\u00a0 The members of the Network include administrative staff of the SBHCs as well as medical personnel such as physicians, nurse practitioners, registered nurses, social workers, psychologists, and health educators from hospitals\/medical centers, community health centers, and private practice.\u00a0 The comprehensive SBHC services are provided by a multidisciplinary team that includes, at a minimum, a nurse practitioner\/physician assistant, registered nurse, physician, mental health counselor, clinic coordinator\/receptionist, and program manager.\u00a0 Being a Program Monitor for a statewide program entails interaction with all of these service providers as well as the administrative staff of each assigned SBHC.\u00a0 Supervision is received from the Program Monitor Supervisor and Medical Director of the Adolescent School Health Program.<\/p>\n<p>ESSENTIAL FUNCTIONS<\/p>\n<p>30%\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 A.\u00a0 Contract Monitoring<br \/>\nNegotiates, develops, and monitors contracts with operating and planning grantees and supervises preparation of their quarterly progress reports.\u00a0 Negotiation of the contracts includes assisting directors in preparation of their budgets as needed.\u00a0 Contract monitoring consists of assessing performance of and monitoring:<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 the types, number and quality of services provided by the SBHCs<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 the minimum staffing requirements and credentials of personnel<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 the minimum enrollment of students in the SBHCs<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 the SBHC&#8217;s adherence to Adolescent and School Health Program policies, procedures, guidelines, standards, and principles as well as Louisiana law<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 staff attendance at activities and meetings<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 financial match of sponsoring agencies and other sources to meet minimum Adolescent and School Health Program requirements<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 status of SBHCs to become Medicaid providers and the revenue collected<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 the SBHC&#8217;s system for 24-hour coverage to ensure access to services when the SBHC is closed<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 the SBHC&#8217;s use of the statewide standard encounter form and uniform data collection system to obtain accurate utilization data for monthly, quarterly and annual reporting<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 the SBHC&#8217;s adherence to the Continuous Quality Improvement program that includes reviewing the annual LAPERT as well as being the program office staff liaison coordinating and participating in site reviews of the SBHCs<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 the monthly, quarterly, biannual and annual statistical, narrative and financial reports submitted by the SBHCs to determine their quality and number of services as well as the continuing relationship between the SBHC, the school, the community and the sponsoring agency<br \/>\nThese functions are accomplished through reviewing reports, site visits, meetings, trainings, and daily contact with the staff of the SBHCs and the sponsoring agencies.\u00a0 Monitoring the contracts also includes working with the grantees to achieve compliance with the contract as necessary.<\/p>\n<p>30%\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 B.\u00a0 Technical Assistance<br \/>\nProvides technical assistance to planning grantees and community sponsors expanding from one to multiple school-based health center sites under one sponsoring agency to assure consistency of staffing, services, standards, and administration; facilitates meetings of provider entities to assure agreement among participating agencies; organizes and conducts trainings for medical providers, behavioral health providers and administrative staff of SBHCs regarding changes in program office standards, procedures for data collection and entry, and how to code (ICD-9 and CPT medical codes) appropriately on the statewide SBHC standard encounter form.\u00a0 Coordinates trainings for SBHC staff on best practices in school-based health care.<br \/>\nProvides one-on-one technical assistance to staff members of operating school health centers including assisting staff in adapting to the e-mail system, new reporting forms, and other changes; guides the staff through State requirements; helps staff with solutions to problems including getting them in touch with appropriate resources and personnel; involves the staff in trainings and meetings which will benefit them.<\/p>\n<p>15%\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 C.\u00a0 Communications<br \/>\nProduces and assists in producing materials for the Adolescent and School Health Program, such as:<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 the Adolescent School Health Annual Services Report, an extensive report which explains the scope of the program, provides valuable statistics, and illustrates that the Adolescent and School Health Program is accountable for program funds<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 special documents to assist the Network in accomplishing its goals (for example, fact sheets, reporting forms, etc.)<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 broad based informational documents that educate about adolescent health<br \/>\n\u0082\u00a0 \u00a0\u00a0\u00a0statistical or informational documents that have been requested by legislators, policy makers, medical personnel, and others<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 Call For Proposals<br \/>\nPresents information about the Adolescent and School Health Program as necessary.<\/p>\n<p>10%\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 D.\u00a0 Liaison to Other Agencies<br \/>\nServes as a liaison with agencies and\/or committees for the purposes of furthering the goals of the Adolescent and School Health Initiative including, but not limited to:<br \/>\n\u0082\u00a0\u00a0\u00a0\u00a0 co-chairperson of one of the Adolescent and School Health Advisory Subcommittees<\/p>\n<p>5%\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 E.\u00a0 Participates in the review of grant applications and contract renewals and communicates decisions to applicants in conjunction with the Intergovernmental Coordinating Council.<\/p>\n<p>5%\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 F.\u00a0 Participates in planning programs for the meetings of the Adolescent School Health Center Network; invites speakers on topics relevant to the continuing expansion and development of school health center services; attends local, state, and national meetings to obtain and disseminate information to members of the Network;
