{"id":140,"date":"2008-06-02T12:41:32","date_gmt":"2008-06-02T17:41:32","guid":{"rendered":"http:\/\/mchtulane.wordpress.com\/?p=140"},"modified":"2008-06-02T12:41:32","modified_gmt":"2008-06-02T17:41:32","slug":"community-networks-coordinator","status":"publish","type":"post","link":"https:\/\/thatscourtney.space\/blueprint\/community-networks-coordinator\/","title":{"rendered":"Community Network Coordinator"},"content":{"rendered":"<p align=\"center\"><strong><span style=\"font-family:Arial;font-size:x-small;\"><span style=\"font-size:10pt;font-family:Arial;\">Latino Commission on AIDS<\/span><\/span><\/strong><\/p>\n<p align=\"center\"><span style=\"font-family:Arial;font-size:x-small;\"><\/span><strong>EXTERNAL JOB POSTING <\/strong><\/p>\n<p align=\"center\"><strong>Community Network Coordinator<\/strong><\/p>\n<p align=\"center\"><strong>Effective May 25, 2008<\/strong><\/p>\n<p><strong> <\/strong><\/p>\n<p><strong> <\/strong><\/p>\n<p><strong>PROGRAM: <\/strong>Accion Por La Salud\/ADAP<\/p>\n<p><strong>REPORTS TO: <\/strong>Director of Access to Care Services<strong> <\/strong><\/p>\n<p><strong>PRIMARY RESPONSIBILITIES<\/strong><\/p>\n<ul type=\"disc\">\n<li>Develop      and maintain community relations with HIV providers and Community Base      organization for the referrals of clients.<\/li>\n<li>Participate      in community outreach events to provide information and to link possible      client to Accion: Health fairs, Community festivals, AIDS Walk, LBGT      events, parades, conferences and community based organizations special      events.<\/li>\n<li>Participate      in Tabling and the distribution of program promotional materials at      community events.<\/li>\n<li>Identify      Referrals for enrollments into ADAP, other healthcare coverage and      supportive services<\/li>\n<li>Support      Community Health Coordinator  in the screening and      assessment of clients (Determination of eligibility into a healthcare      coverage program)<\/li>\n<li>Support      in providing presentations on program services<\/li>\n<li>Enter weekly      data in URS and prepare outreach activity report<\/li>\n<\/ul>\n<p><strong> <\/strong><\/p>\n<p><strong>ADDITIONAL RESPONSIBILITIES:<\/strong><\/p>\n<p><strong> <\/strong><\/p>\n<p>\u00d8      Data entry<\/p>\n<p>\u00d8      Enter and maintain all required information in appropriate log(s)<\/p>\n<p>\u00d8      Other Duties as assigned by Program Director<\/p>\n<p><strong> <\/strong><\/p>\n<p><strong>QUALIFICATIONS:<\/strong><\/p>\n<ul type=\"disc\">\n<li>Bachelor&#8217;s      degree in Social Work, Psychology, Community Health or related field with      at least four (4) years experience in HIV counseling and testing.<\/li>\n<li>Four      (4) years Outreach experience in communities of color<\/li>\n<li>Knowledge      of Oraquick and Orasure HIV testing procedures<\/li>\n<li>Excellent      communication skills, writing and verbal<\/li>\n<li>Computer      literate Microsoft Word and Excel<\/li>\n<li>Bilingual      (English and Spanish)<\/li>\n<\/ul>\n<p><strong> <\/strong><\/p>\n<p><strong>Submission of Resume<\/strong>:  Interested candidates submit cover letter and resume to:  Ofelia Barrios, Director of Programs at <a href=\"mailto:obarrios@latinoaids.org\">obarrios@latinoaids.org<\/a> or fax at (212) 675-3466 by June 20, 2008.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Latino Commission on AIDS<br \/>\nEXTERNAL JOB POSTING<br \/>\nCommunity Network Coordinator<\/p>\n","protected":false},"author":3,"featured_media":0,"comment_status":"open","ping_status":"closed","sticky":false,"template":"","format":"standard","meta":{"footnotes":""},"categories":[5],"tags":[],"class_list":["post-140","post","type-post","status-publish","format-standard","hentry","category-general-posts"],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v27.6 -
